Community Health and Wellbeing Worker
Location: Stonehouse - Plymouth
Salary: £13.31 hourly rate
Hours: 20 hours per week – Monday to Wednesday 09:00-15:00 / Thursdays 09:00-12:30
Contract: Fixed Term until 31st March 2027
This is an exciting opportunity to join an innovative programme that is at the forefront of improving community health and reducing health inequalities in Stonehouse, Plymouth.
The Wolseley Trust’s Community Health and Wellbeing Worker (CHWW) programme is a partnership with Waterside Primary Care Network, and supports residents to access primary care, navigate local services, and connect with organisations that can enhance their health and wellbeing. This role is part of a national movement to strengthen community-based care, led by the National Association of Primary Care (NAPC) and pioneered by Dr Matthew Harris, and aligns with the ambitions of the NHS 10-Year Plan.
As a CHWW, you will work directly within your local community, identifying unmet needs, building long-term trusted relationships, and providing a bridge between community, health, and care services. Your role is central to improving access to care, supporting people to overcome barriers, and empowering residents to make choices that enhance their wellbeing.
You will need confidence, resilience, and a coaching approach to engage with individuals and households, helping them navigate challenges and access the right services for their needs. Insight into the lived experiences of your community—whether through personal experience, volunteering, or professional work—will help you develop empathetic and effective relationships. You will work inclusively and in a trauma-informed way, respecting the diverse needs of the people you support.
