Job Vacancy - Social Prescribing Link Worker

The Wolseley Trust is recruiting a social prescribing link worker to join our ‘Healthy Futures’ social prescribing team. Healthy Futures is delivered in partnership with Primary Care Networks and VCSE organisations across Plymouth.
Social prescribing enables the utilisation of community resources to meet needs unmet by traditional health service delivery models by developing relational networks between local services which enable health and wellbeing.
Working as a member of the Wolseley Trust Social Prescribing team and teams within Primary Care Networks, you will use the social prescribing model to help people find appropriate community and non-clinical solutions to their needs; to reduce their social isolation and improve their health and wellbeing.
As a Social Prescribing Link Worker, you will have the opportunity to work directly within Waterside Health Network surgeries, becoming part of the wider primary care team and building strong relationships with patients, clinical colleagues and community services.
You will have excellent holistic assessment, motivational and consultation skills, and will ideally have undertaken formal training in relation to health; social care, coaching or therapeutic approaches, or have other relevant professional or academic qualifications or experience.
Fixed-term until March 2027, reflecting the current commissioning period. We have longstanding partnerships with our Primary Care Networks (PCNs) and a strong track record of contract renewals and continued service delivery. While any extension is subject to future commissioning arrangements, there is a positive history of ongoing partnership working.
